PLEASE PROVIDE THE FOLLOWING DOCUMENTS AT LEAST 7 DAYS PRIOR TO THE MEETING OF CREDITORS
Please upload the following documents to my office using the Stretto document portal. If you need an invitation to upload to the Stretto document portal please contact Lori Werner at lwerner@go2.law
1. The most recently filed state and federal tax returns.
2. A copy of a valid, government issued picture identification card (i.e., state issued driver’s license or identification card, or a passport). Please note that the copy provided must be in COLOR and the image CLEARLY VISIBLE.
THE MEETING WILL NOT GO FOWARD IF THIS IS NOT PROVIDED PRIOR TO THE MEETING
3. A copy of a social security card, Medicare card, W2 or 1099 wage statement bearing the debtor’s entire social security number, employer identification number or tax identification number.
4. If the Debtor pays spousal support or child support, please have them complete the Domestic Support Obligations (DSO) Form – by providing the name, address and telephone number of the support recipient. When completed email to lwerner@go2.law
Attorneys - You will be asked to please confirm that you have personally verified the Debtor's identity and the identification documents and to confirm that the documents match the name of the Debtor and the identification number that appears on the notice of meeting of creditors.

IMPORTANT: Please note that the 341(a) Meeting of Creditors are subject to being reset IF Trustee Golden is contacted by a creditor who wishes to examine the Debtor after the video examination or telephonic examination of the Debtor has already taken place.
