PLEASE PROVIDE THE FOLLOWING DOCUMENTS 14 DAYS BEFORE YOUR MEETING OF CREDITORS (AND IN NO EVENT LATER THAN 7 DAYS BEFORE THE MEETING)
The following documents must be electronically sent to me via the following email address bokinskie@portersimon.com:
1. Proof of ID and SSN. This requires CLEAR copies of original proofs of identification and Social Security number. If proof of ID and/or SSN is missing the debtor will not be examined, and the meeting of the creditors will be continued. You cannot show me the documents on the screen during the 341. Please note that the copy provided must be in COLOR and the image CLEARLY VISIBLE

2. Tax Returns. A complete copy of federal and state tax returns. If the Debtor(s) has not filed returns for several years, please indicate.
3. Pay Advices. All pay advices for income, or other receipts, received by the debtor(s) for the period 60 days prior to the filing of the petition. If debtor(s) have many deposits for amounts other than shown on pay advices please explain the source.
4. Financial Accounts. Current statement for every financial bank account maintained by the debtor(s) covering the 30-day period to and including the petition date. A "statement" is not screen shots or photos from a debtor's phone. A failure to provide a statement including the petition date (the date the bankruptcy is filed) may result in a continuance of the meeting of creditors.
5. Retirement Assets. Current statement for every retirement asset.
6. Lien Statements and Kelly Blue Book. Private party value (NOT trade in value) for each vehicle, trailer, or other personal property capable of a private party valuation by KBB or NADA. Also include current account statements evidencing any lien, and the amount thereof, against any property of the debtor's estate.
7. Life Insurance- Term and/or Whole Life. Current statement for every insurance policy.
8. Annuities. Current statement for every annuity.
9. Domestic Support Obligation Checklist (if applicable). If you pay spousal support or child support, please complete the Domestic Support Obligations (DSO) Form – by providing the name, address and telephone number of the support recipient. This should be provided as a 521 document BEFORE the 341 date. DSO Form.pdf
10. Spousal Waiver (if applicable).
11. Statement of Debtor Not Represented by an Attorney (if applicable). If you are a debtor and you do not have an attorney, you must provide a completed, signed and dated copy of the DECLARATION OF DEBTORS WITHOUT AN ATTORNEY. When completed, please email to bokinskie@portersimon.com

REMINDER: In order to obtain a discharge of debts, an individual debtor (not a business debtor) must complete an approved course in Personal Financial Management within 60 days after the 341(a) Meeting of Creditors. Make sure this is filed with the Bankruptcy Court or you may not receive a discharge.
IMPORTANT: Please note that the 341(a) Meeting of Creditors are subject to being reset IF Trustee Birnberg is contacted by a creditor who wishes to examine the Debtor after the video examination of the Debtor has already taken place.
