Important Deadlines to File PIQ:
PIQ Response: Submit by August 28, 2023
Supporting Documentation: Submit by September 26, 2023

If you wish to file your PIQ electronically, please review the cover letter mailed to the address on file with your claim and locate your password.

Additionally, if you provided an email when filing your Asbestos Claim form, please submit it below, and you will receive your password in an email shortly.

Asbestos Claimants can also DOWNLOAD the Asbestos Claimant Personal Injury Questionnaire, Asbestos Trust Release, and Medical Record Release. Upon completion of a PIQ form (together with any supporting documentation you may have), please remit to the following address:
